From your description there is no meat left on your clutch plate or adjustment, replace the clutch and it should fix your problem.

there is a heavy duty option through Adelaide clutches (I think they are called extreme clutches from memory).

Master cylinder does both brakes and clutch so make sure it is good if you have replaced with a second hand one.

You may need to go genuine on that one.
