97 SR transmission question

97 Sr 230k Fluid looks good ( unsure of brand as po had it changed )
Had a question about shifting and or granny fear of some sort
When leaving a stop light, I often drive like a grandpa haha. It seems when I do this (just enough throttle to get her going around town etc) the transmission feels like it shifts around 5-10 mph (no shift occurs, as the rpms continue to rise and shift into 2-4 as I continue on)
If I give a bit more throttle (say around 1/4 pedal ) it doesn't have the shift feeling and continues into 2-4 as always
The only other instance that I feel any modification from the "normal" 1st gear is when climbing a steep hill in D, but transmission in 1st. If I stay off the throttle a bit the car seems to downshift and gains more torque to pull me up the hill
Thoughts?

Have you check the cable adjustment per the FSM?
