Where your truck might be different, is that I think most V-6 trucks came with ADD. But I'm guessing yours did not. I'll bet you have manual hubs. Your set up is already unusual in the fact that you have 4.30 diff gears and yet a manual tranny. The 4.30 gears are usually reserved for automatics with the smaller 225/75R-15 tires. And speaking of that, I assume original tires on your truck were 225/75R-15? If so, then your gear ratio may really be 4.10, or if it really is 4.30, then you must have some sort of rare tow package. Do you have a factory or dealer installed hitch or wiring?
If it is possible to have a neutral in a factory chain driven T-case, then the likely explaination is that Toyota just installed neutrals in T-cases without ADD as indicated by my owner's manual. On the other hand, if neutral were not possible in a chain driven T-case, then I assume, you must somehow have a gear driven T-case.

Yep, I have manual hubs. Original tires were 225/75/R15. I have a "tow package", whether it is from the factory or not I don't know. It has a ball on the bumper, but the wiring doesn't really look all that "factory".
As far as the gears, I've double checked by lifting one tire and spinning. I'll have to do that again though.
I have not confirmed the t-case by the bolt numbers, I'll have to check that next.
