Just got the truck back after having the head replaced. Found the surging to be a small crack betweent he valves on cylinder 3. Now it runs great. Except when I first go to start it in the morning. It doesnt go to high idle. As soon as I let go of the key it dies. If it put my foot on the gas and hold the idle up for about 20 seconds it runs fine. Once its warmed up its fine for the rest of the day.

Any ideas? I swear I read something just like this somewehere but I cant seem to find it no matter how much I search.

they obviously did not connect something. go searching for a wire or hose that is not connected. you already knew that though and probably did that. a bunch of those hoses and wires are all related to idle. just because a wire looks like it is hooked up, it may not be. my brother's had a CEL for P0340 - cam position sensor. it turned out that they must have undone the connection in front of the timing belt cover to armor all the engine plastic stuff and did not fully put it together when they were done. it looked like it was together, but when i pushed it finally clicked into place. CEL went away. saved us $200 because the dealer thought the cam position sensor was bad. it would be pretty damn hard for one of those to fail.
