i had a similar thing happen on my first car, a '72 Plymouth Duster. the bearing(s) in the fan clutch apparently went bad and sent the fan into the radiator. so, being a goofy 16 y.o. kid, i replaced the radiator and fan, and it happened again. my uncle then told me to replace the fan clutch, and that solved the problem.
