Where can I buy a bolt up replacement cat or, bolt up replacement pipe for my 86 sr5? The one on it now looks like the original from 86. 220k mile later, it must be bad.

Why you think its bad? When you put your hand over the tailpipe while at idle...do you feel pressure from the exhaust pipe?
220K miles aint chit. Hell I had over 220K on my 88's cat when I pulled it, looked as good as new! Nice white honeycomb from one end to the other. I bolted it onto my 87 4Runners exhuast and its still going strong.
If ya wanna toss away good money go buy some bling for you rig. I hear a carbon fiber shifter and some of them racing pedal knock offs will add an easy 50 HP

Summit Racing has a good selection of affordably priced cats, including one that is supposed to be a drop in replacement on your truck. Though I have heard it isn't as good a fit as claimed.
To assume your cat is no good due to age isn't on the mark. You can remove it and inspect it to see. That would cost you only the two gaskets involved. Then you would know if your need a new cat or not.

Replaced the cat earlier this year to pass inspection, the rear corners were rusting out and leaking exhaust. I picked one up at the local foreign car parts store -- the box said A B Import Parts out of New Jersey. The dealer had quoted $700, local place sold it to me for $150. You could not tell the difference between the two (other than the 175K and 18+ years on the original one). They were identical. Bolted up with no problems.

I just got one for my 1993 22R-E from these guys: Cat Converter The price is very good at $100. Thier shipping is $25 or so, I called and they took $10 off for no reason. The cat bolted right on, but the gaskets they provided leaked. I ended up using my old Toyota gaskets and things are 100%.

If you want to go through the trouble of replacing it, you should at least inspect it first before you buy a new one. Japanese cats are constructed like a hollow brick of fine square passages. I replaced mine (had to...it failed smog testing at around 125k). The muffler shop fella showed me the cat and the brick was 80% missing! I guess it fell apart and blew into the muffler. I got a hi-flow unit and went 2-1/4" aluminized tubing with a muffler and haven't had any cat/smog issues since.

Not that expensive, so I would not bother trying to find cheap gaskets...

Where exactly does the cat go? Upstream or downstream of the o2 sensor?
My exhaust was so rusted up that the mount for the 02 was about to come loose, so I had an exhaust shop in east TN (where there are no emissions) weld me up a new bung.
I took it to a shop here locally where emissions are required, and the shop wouldnt touch it cuz it didnt have a cat. He wanted like $300 to put one on there.
I would like to get it legal one of these days.
Its registered in east TN now so I dont have to worry about it... for now.

If you only have one O2 sensor, it goes after the sensor. If you have two sensors, it goes between them. Usually the cat ends at the center exhaust hanger, its flange has hooks that go to the rubber donuts off the hanger.

If it needs replacing, I got my new one from NAPA. Bolted right up, came with new gaskets, $124.
