I've had the exact same thing happen to me. It only happens in first or second gear and only when I am revving the engine for a faster start. I thought it was the t/o bearing so I replaced that and the pilot bearing (the clutch components only had about 10K on them). The noise went away for about 8 months but is now gradually coming back.
