I've changed a few 6VD1 belts and all looked brand new, even the first one with just over 130K on it.

The timing belt tensioner is what gets weak, causing the belt to slacken and possibly jump a tooth.

While the belt is out, you may as well do the water pump unless you just love tearing down the front of the motor.
