Perhaps this picture will help. There are 2 rubber grommets you remove from the brake backing plate. One allows access to a retainer (which you push out of the way), the other to a star wheel which you use to adjust your shoes:

I prefer to remove the tire/wheel and the drum itself to not only adjust properly, but also clean things up well so they can self-adjust.
