I quoted the book, except the 2h-4h-2h shift on the fly because I forgot it, since I've never had manual hubs mounted. I actually prefer to shift mine at a very slow roll, with all bind off the drivetrain, foot off the brake, a/t in neutral, and since I usually never shift into 4wd until I need it, I'm often not moving at all....
