I dont have a runner, just xtended cab, but I think you might be having power issues that are just problems. First check that TPS to make sure it is telling your engine to run right, that thing seems to screw up everything if it is off. Also check your vacuum hoses, replace them, they are cheap, and I noticed a big difference in how smooth my engine ran. Check your 02 sensor, those can force the engine to run rich if they are off. Check you PCV grommet. If your TPS if bad, get a new one, tune it up and it will run like a new engine.
