1 & 2, I usually reuse those (depending on condition of gasket), but you can replace them if you want. I just hammer the washer flat and/or flip it over. This works for a few times anyway.
3-6, yes. I'd recommend Brembo rotors over OEM. Tire-rack.com is a good source.
7, Hard to beat this price: http://trail-gear.com/knuckle-tools.html but a 2-1/8" one locally might be an option as well. 54mm or 2-1/8" may be tough to find w/o some searching though, and you usually need to also buy a 3/4" to 1/2" adapter to use it.
8, you shouldn't need them. I can't recall ever replacing one on a Toyota that wasn't severely abused/neglected.
