I'll just paste my response & info i have, also why don't u use the clutch parts from your old cab?

PARTS U NEED:
1.Tranny & Tranfer
2.Clutch pedal assy.
3.Clutch master cyl, slave cyl and hard&flex line
4.Shifter surround for inside the cab

You'll have to take good measurements from the donor truck to get the holes in the firewall right for mounting
the clutch master cyl.
I don't remember what size bi-metel hole saw you need, maybe someone else does.
You need to cut a longer opening in the floor for the shifters.
You don't need another ECU and you're drive shafts will fit fine.
All autos i've seen have had a gear ratio of 4:30 and factory 5-speeds have 4:10s.

You'll also have to fix the wiring harnees so the truck will still start.
Now in the big group of plugs that hook to the auto theres a white round one that has three wires,
two big wires and one small. You have to wire the two big ones together.
On mine i threw the auto in the junkyard so i cut the half off the tranny(leaving a few inches of wire on it)wired
the big ones
together cut the small one off and pluged it back into the truck, works great. You have to becareful though because
the truck will try to start whether the clutch is in or not.

EVERYTHING WHAT HE SAID
you will also need the flywheel pilot bearing flexplate cover from the donor rig. get a shifter boot and cut the floor corisoningly to the base plate..or do what i did and cut the tranny hump out of the donor cab and rivet it onto the existing floor boards. the wire is the big black and black with white strip...you can also run those wires up the the clutch switch so it wont start in nuetral...also you will see that the harness from the 4wd auto comes apart for the tranny and transfercase..the transfercase harness will plug right in to the 4wd 5speed transfercase connector.you can make the crossmember from the auto work but if you have a donor rig just take the crossmemberand mount from that... you will need to drill a 1" hole through the firewall but bolt the brake/clutch assembly up first to mare the hole first.
Its super easy to do the swap and it only took me an afternoon to pull the auto out and pull the 5speed out and install the 5speed in the good truck.
