If so, item 5 is part number 8-94442-398-0 and item 6 is part number 8-94442-399-1. The 3 screws have part number 9-93090-408-0. Double check on the screws part # with whoever you order from. I ordered the screws and when they arrived, the heads of the screws were rounded and would not sit flush. The screws you need are flat headed machine screws.
