As dumb as it sounds, my plugs have something to do with this. I change plugs and it starts and runs fine for around 500 miles. Then I have it running after 500 miles and it is cold and choppy in the morning. My 2 cents.

try reading the codes with a cheap 12v test light. Long blinks are the first number in the two digit code, short blinks are the second number. For example, long blink, long blink, short blink is 21. Some voltmeters (like a good digital or an undamped cheapie) bounce too much to get a good reading. The light works better.

Also check the accordion hose for any leaks. Air leaks there bypass the mass air sensor, and can lean out the mixture enough to make it run bad cold. Check the hose to the valve cover off the accordion hose, too, for leakies.

Your problem sounds a lot like the one I fought for a LONG time. It turned out I had two vacuum lines backwards.

When the engine is cold, start it up and go to the EGR valve. Pull on the diaphragm and see if it acts any differently. If it does, you have a vacuum line on wrong somewhere. Mine turned out to be on the back of the intake plenum. If you look at the vacuum hose diagram, you'll see two fittings up there. Once I corrected the vacuum connections, it immediately started running correctly and passed CA smog the first time with flying colors. I promise you that it's the simple things that get you, and you don't want to know how much time/trouble that little vacuum hose cost me.
